1. I declare that all the information given in this form is true and complete to the best of my knowledge. 1. 本人谨此声明在申请表内填报的资料均属正确及完备。本人明白若提供任何虚假或误导性资料，本人的申请资格将被取消，虽经注册，亦属无效，圣方济各大学会把任何涉嫌入学欺诈的个案转交执法部门处理，并保留采取其他行动的权利。于此情况下，所缴费用一概不发还或转让他人。

Notes:

1. Applicant must complete this application via Saint Francis University Online Admission Application System from 16 January (9:00 AM) to 6 March 2023 (5:00 PM). Copies of the HKID Card / Passport, academic transcript(s) with examination results, certificate(s) to prove that the applicant has fulfilled the entry requirements, additional document(s) e.g. proof of working experience in welfare sector or recommendation letter(s) from the welfare organisation(s) (if applicable), must also be submitted.
2. Applicant who is currently a full time staff in the welfare sector in the time of application, should provide documentary proof showing the name of the employing organisation, the name of the service unit where the applicant is working, the position held and the full time nature of the employment. Application without the above documentary proof will not be accorded priority in the shortlisting and selection process.
3. For education attainment obtained outside Hong Kong, applicants are required to submit to Saint Francis University documentary proof of its Hong Kong equivalent; otherwise, the Social Welfare Department and Saint Francis University will not be able to consider that education attainment.
4. Applications will not be processed under any of the following circumstances –
   (a) not submitting the application via Saint Francis University Online Admission Application System;
   (b) applicant has not attained the age of 18 on 11 September 2023;
   (c) application with no attachment of copies of academic certificates; and
   (d) late application.
   
5. The personal data collected in this application form will only be used for the purpose of processing the application for admission to the training programme. For unsuccessful applications, the application form and all the information and documents as submitted by the applicants will be destroyed in 24 months following the commencement of the training programme.

1. 申请人必须于2023年1月16日（上午九时正）至3月6日（下午五时正）或以前， 在圣方济各大学网上入学申请系统递交申请。申请人在网上递交申请时必须同时提供香港身份证／护照副本；附有考试成绩的成绩单副本；证明符合指定入学条件的证书副本；其他证明文件，包括任职社福界的证明／社福机构推荐信（如适用）。
2. 现时全职在社福界工作的申请人，所提供的证明文件须显示其现时受雇的机构名称、服务单位名称、职位名称及其为全职雇员身份。未有提供上述文件的申请，在遴选过程中将不获优先考虑。
3. 如申请人在香港以外的地方取得学历，须向圣方济各大学递交证明其等同于香港学历的证明文件，否则社会福利署及圣方济各大学将无法考虑该学历。
4. 在以下任何一种情况下，申请将不获处理 -
   (a) 未有于圣方济各大学网上入学申请系统递交申请；
   (b) 申请人于2023年9月11日尚未年满18岁者；
   (c) 未有提供学历证书副本；及
   (d) 逾期递交的申请。
5. 本申请表内所收集的个人资料只会用作处理申请报读本训练课程之用。未能成功入读课程的申请人所提交的申请表以及申请人提交的所有资料将会在课程开学24个月后全部销毁。
6. 提交本申请表后，如欲查阅或更正个人资料，请在办公时间内联络圣方济各大学(电话：3702 4388)。

I understand that this information will be used for processing my application for Higher Diploma in Enrolled Nursing (General) programme which will be commenced on 11 September 2023, and any misrepresentation may lead to disqualification of my application for admission and cancellation of subsequent enrolment.

I have read through the Guide to Applications of the Higher Diploma in Enrolled Nursing (General)- Enrolled Nurse (General) Training Programme for the Welfare Sector 2023-24 and understand the content therein.

I understand that if I apply for the above-mentioned Higher Diploma in Enrolled Nursing (General)- Enrolled Nurse (General) Training Programme for the Welfare Sector for the Welfare Sector, I have to simultaneously apply for the Higher Diploma in Enrolled Nursing (General) run by Saint Francis University.

I hereby give consent to the Social Welfare Department to obtain from/release to Saint Francis University my relevant personal information including but not limited to education level, results of public examination, academic qualifications, working experience and interview results. I also understand that this application is to be collected via Saint Francis University Online Admission Application System and Saint Francis University will pass to the Social Welfare Department for processing the admission application.

本人已知悉并明白如本人申请上述普通科护理学高级文凭-社福界登记护士（普通科）训练课程，即本人必需同时申请圣方济各大学举办的普通科护理学高级文凭课程。

本人同意社会福利署向圣方济各大学索取／发放本人相关的个人资料包括但不止于教育程度、公开考试成绩、学历、工作经验及面试结果。本人亦明白本申请由圣方济各大学网上入学申请系统代为收集，并会由圣方济各大学交予社会福利署作处理入学申请之用。
